Анализ проводили в когортах с уровнем иммуносупрессии &lt;200, 200–499, &gt;500 клеток/мкл. Временной горизонт исследования – 2 года. Оценивались три алгоритма: «Базовый» – последовательность стандарт- ных этапов диагностики ТБ в РФ, «Дополнение» – «Базовый» алгоритм дополнен Gene Xpert MBT/Rif; «Замена» – только Gene Xpert MBT/Rif.</p></sec><sec><title>Результаты</title><p>Результаты: использование алгоритма «Дополнение» в группе пациентов с CD 4+ &lt;200 кл/мкл позволяет дополнительно выявить 8 больных ТБ на 1000 ВИЧ-инфицированных, значительно увеличить число эффективно пролеченных пациентов, в том числе среди пациентов с исходной лекарственной устойчивостью к рифампицину, предотвратить 5 летальных исходов, уменьшить вероятность передачи ТБ контактам и дальнейшее распространение ТБ в популяции. Дополнительные затраты на 1 эффективно пролеченного пациента, выявленного в рамках алгоритма «Дополнение», по сравнению с «Базовым», с учетом популяционного эффекта диагностических пропусков, составят 1130 тыс. руб., дополнительные затраты на 1 предотвращенный летальный исход – 943 тыс. руб. Алгоритм «Замена» может привести к значительным диагностическим потерям, и его использование не имеет ни клинической, ни экономическойцелесообразности.</p></sec><sec><title>Выводы</title><p>Выводы: добавление Gene Xpert MBT/Rif к алгоритму поиска ТБ экономически оправданно в когорте пациентов с уровнем CD 4+ &lt;200 клеток/мкл.</p></sec></abstract><trans-abstract xml:lang="en"><p>Diagnostic of active tuberculosis among HIV – infected people is difficult problem, demanding of significant healthcare costs.</p><sec><title>The objective</title><p>The objective: to evaluate the cost-effectiveness of different diagnostic scenarios detecting tuberculosis among HIVinfected.</p></sec><sec><title>Material and methods</title><p>Material and methods: the evaluation was carried out through modeling, from the health care system perspective, in cohorts with different CD 4+ counts (&lt;200, 200–499, &gt;500). The three scenarios were examined: (1) «Base» – the current TB diagnostic scheme in the Russia; (2) «Addition» – the «Base» scenario augmented by Xpert/Rif; (3) «Replacement » – only one Gene Xpert/Rif.</p></sec><sec><title>Results</title><p>Results: using «Addition» scenario in CD 4+cohort &lt;200, allows to detect 8 additional TB patients per 1000 HIV-infected people, significantly increases the number of effectively treated patients, including patients with initial rifampicin drug resistant, prevents 5 deaths, reduce opportunity oftransmission TB and further spread TB in population. Additional costs per one additional effectively treated patient, identified in the «Addition» scenario, compared to «Basic», is 1130 thousand rubles, additional costs per one prevented death is 943 thousand rubles.
