Проведен анализ течения коронавирусной инфекции (COVID-19) у 16 больных с ВИЧ-инфекцией, находившихся на стационарном лечении в Центре по профилактике и борьбе со СПИД и инфекционными заболеваниями с апреля по октябрь 2020 г. Всем больным проведено исследование биологического материала из рото- и носоглотки на COVID-19 и поставлен диагноз на основании положительного результата ПЦР.</p></sec><sec><title>Результаты</title><p>Результаты. У ВИЧ-инфицированных больных с диагностированной коронавирусной инфекцией, вызванной COVID-19, установлены признаки прогрессирования ВИЧ-инфекции (75%), клинические, иммунологические, вирусологические, оппортунистические заболевания и сопутствующая патология (хронические вирусные гепатиты в цирротической декомпенсированной стадии, сердечно-сосудистые заболевания и др.)(94%). Малая выборка больных не позволила с достоверной точностью определить взаимовлияние имеющихся заболеваний и патологий, но, безусловно, множественная коморбидная патология играет роль в развитии тяжелых состояний и неблагоприятных исходов. Представлен клинический случай.</p></sec><sec><title>Заключение</title><p>Заключение. Выявлены провоцирующие факторы, играющие роль в развитии инфицирования и более тяжелых форм коронавирусной инфекции, вызванной COVID-19 у ВИЧ-инфицированных пациентов: употребление инъекционных наркотиков, злоупотребление алкоголем, поздняя стадия ВИЧ-инфекции (4Б, 4В) и прогрессирование более ранних стадий (4А), низкое количество CD4-лимфоцитов (менее 200 кл/мкл), множественная коморбидная патология (ВИЧ-инфекция, оппортунистические заболевания, сопутствующая патология, особенно хронические поражения печени в стадии декомпенсированного цирроза), отсутствие терапии, прерванная антиретровирусная терапия, множественные смены схем, отсутствие профилактики оппортунистических заболеваний). У больного с ВИЧ-инфекцией в стадии 4В с множественной коморбидностью установлена возможность длительной персистенции коронавируса COVID-19 с получением положительных и отрицательных результатов в течение более 2 месяцев и позднее развитие поражения легких, вызванного COVID-19. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose. Analyze and identify the features of the course of the new coronavirus infection (COVID-19) in HIV-infected patients.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An analysis of the course of coronavirus infection (COVID-19) was carried out in 16 patients with HIV infection who were hospitalized at the St. Petersburg State Budgetary Healthcare Institution Center for the Prevention and Control of AIDS and Infectious Diseases from April to October 2020. All patients underwent a study of biological material from the oropharynx and nasopharynx for COVID-19 and diagnosed based on a positive PCR result.</p></sec><sec><title>Results</title><p>Results. In HIV-infected patients with diagnosed coronavirus infection caused by COVID-19, signs of progression of HIV infection, clinical, immunological, virological (75%), opportunistic diseases and comorbidities (chronic viral hepatitis in the cirrhotic decompensated stage, cardiovascular diseases and others) (94%). A small sample of patients did not allow to determine with reliable accuracy the mutual influence of existing diseases and pathologies, but, of course, multiple comorbid pathologies play a role in the development of severe conditions and unfavorable outcomes. A clinical case is presented.</p></sec><sec><title>Conclusion</title><p>Conclusion. The provoking factors have been identified that play a role in the development of infection and more severe forms of coronavirus infection caused by COVID-19 in HIV-infected patients (injecting drug use, alcohol abuse, late stage of HIV infection (4B, 4C) and progression of earlier stages (4A ), a low number of CD4 lymphocytes (less than 200 cells / μl), multiple comorbid pathology (HIV infection, opportunistic diseases, comorbidities, especially chronic liver damage in the stage of decompensated cirrhosis), absence, interrupted antiretroviral therapy, multiple changes of regimens, absence prevention of opportunistic diseases).
